1. Schedule an Access Intake appointment
- Your educational goals
- Academic load needed (full time/part time)
- How your disability affects you in a college setting
- What documentation you need
- What accommodations you used in the past
- What accommodations will you need for placement tests
- Request accommodations:
- Must not fundamentally alter the nature of the program activity or service
- Cannot give an unfair advantage
- Must be directly related to your disability
- Must have a reasonable opportunity to provide equal access
4. Schedule a time to set-up accommodations
- Discuss accommodations for which you are eligible
- Learn the policies and procedures for accessing your accommodations
- Learn about the time and processes needed for requesting accommodations such as notetakers, scribes, etc.
- Meet the Director of the Center for Academic Success
2. Provide Documentation (may be provided prior to the intake appointment)
- Provide documentation of your disability to the ODS that is:
- Current (less than three years old)
- Based on adult assessment and norms
- Signed by a qualified licensed professional
- Shows a substantial limitation to a major life activity
- Supports appropriate accommodations that provide equal access
Documentation may be sent to the ODS by:
- Signing a Release of Confidential Information Release Form then
- Having the documentation faxed, scanned or emailed or mailed from a professional evaluator or agency or by
- Hand delivering the documentation
5. Initiate accommodations for each course
- Pick up accommodation memos from the Coordinator of Disability Services or CAS Director at the beginning of each semester. You should have enough copies for each professor of every course in which you are enrolled.
- Make an appointment at the beginning of each semester (within the first 2 days of classes) to deliver the memos and discuss your accommodations with each instructor. It is your responsibility to notify your instructors of your learning needs.
- Remind instructors at least 3 days in advance if you will be taking a test in an alternative location so that your test will be at the Center for Academic Success and ready during the class session.
- Inform the Director of the Center for Academic Success (CAS) when you will be in need of the testing accommodations at least two days in advance (that includes a reader, a scribe, a separate location or extended time).
3. Documentation Approval Process
- Documentation will be assessed by the
- Coordinator of Disability Services. It may take up to two weeks to process and approve documentation.
- You will be contacted regarding the results of the documentation review
- If documentation is approved, set up an accommodation appointment
- If more documentation is needed, you will be informed about what you need to provide in order to qualify for accommodations
6. Continue to receive services by
- Emailing the Director of the Center of Academic Success (CAS) as soon as you have your semester schedule.
- Stop by the CAS at the start of each semester to pick up your accommodation memos.
- Meet with your professors and discuss your needs
- Remember that each person in the CAS is available to help you - but you must avail yourself of their services in a timely manner.
- Feel free to meet with the Office of Disability Services Coordinator if you are having difficulty meeting your academic goals – a new look at approved accommodations or support may be needed.
